Summary:
 
PATH Ventures is seeking an intern who is fluent in the Spanish language to assist our teams in providing linguistically-relevant services to a large monolingual Spanish-speaking population.

The Intern offers recovery-oriented services in a permanent supportive housing setting. Our tenants are older adults with chronic health and mental health conditions. Services include conducting initial assessments, developing individual goal plans, crisis intervention, increasing access to benefits/healthcare/mental healthcare, community integration, and community development within the apartment community. This Intern will work collaboratively with case management staff and a range of community service providers to coordinate and ensure tenants living in PATH Ventures Permanent Supportive Housing (PSH) Developments are stabilizing in housing, improving the quality of their lives, and integrating into the community. Services will be delivered onsite at the apartment community, in the tenant’s homes, and in the neighboring community. 

PATH Ventures’ permanent supportive housing developments use a low-barrier, Housing First approach coupled with culturally-appropriate, ongoing education and supportive services tailored to the population and  focused on the whole person in order to maximize residents’ physical, economic, and emotional well-being. Interns will practice implementing evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, and Harm Reduction approaches.
 
Current Host Site:
 
  • Winnetka Village: A 94-unit community that serves older adults who either have chronic physical health or mental health conditions. Hosts up to 2 interns.

Sample duties include, but are not limited to:
  • Engaging and building rapport with residents
  • Meeting regularly with residents to coordinate case management services
  • Providing translation assistance to onsite staff while coordinating case management services
  • In conjunction with the resident, developing an Individualized Service Plan that details clear housing and health goals with a plan of working together
  • Performing home visits in conjunction with case management staff
  • Co-facilitating onsite workshops/groups that promote life skill development and community development, and translating as needed.
  • Identifying neighborhood resources that meet the needs of residents
  • Coordinating with community providers to present information on their resources to residents
  • Translating documents and flyers into Spanish to enhance access to information in tenants’ primary language.
  • Documenting every interaction with residents in a GIRP formatted case note.
 
Expectations
  • Maintain a consistent schedule in the office between Monday through Friday. Days/times are flexible, but require consistency.  A PATH Ventures workday is 8 hours with a ½ hour lunch.
  • Ensure the security of confidential information according to HIPAA standards.
  • Have a positive attitude, good sense of humor, and a desire to learn
 
Skills
  • Able to speak/read/write in Spanish, as many of our tenants are monolingual Spanish speakers
  • Ability to provide community and in-home based services on a regular basis.
  • Must demonstrate high level of tolerance and understanding for individuals who present active symptoms with complex case management and healthcare needs.
  • Strong verbal and written communication skills
  • Proactive and a self-starter
  • Excellent attention to detail
