Summary:

The Clinician works as part of the Whole Person Wellness program’s multidisciplinary team to provide clinical support to individuals in the program. The Clinician will be responsible for completing biopsychosocial assessments, solution focused treatment including individual therapy directly in the field and crisis intervention when needed. As part of the team the Clinician will attend community and home visits, assist with community integration and improving quality of life for program participants.

The Clinician will also assist in developing weekly case conferencing to go over the clinical needs of the program participants and will work directly with the program staff to develop triage and acuity tools to ensure the appropriate level of care for each individual.

Supportive Services

  •  Perform initial comprehensive bio psychosocial assessment; develop a plan of intervention that addresses identified need for housing, on-going case management services, substance abuse treatment, mental health services, and coordination of needed community services.
  • Develop individualized care plan in collaboration with client addressing short term and long term goals. Provide on-going case management and individual therapy to support to assess progress and ensure treatment plan outcomes are met or changed as needed.
  • Meet at least weekly with clients to provide therapeutic supportive case management services addressing mental health, functional, and substance use barriers as needed to help clients obtain and maintain housing.
  • Conduct crisis and suicide risk assessment, provide appropriate interventions as needed and consult with LCSW supervisor, complete critical incident reporting with 24 hours as required per contract and PATH policies.
  • Provide crisis intervention services focused on enhancing the clients’ ability to independently problem solve, utilize effective coping skills, and manage and self -coordinate own care.
  • Provide intentional case management supports that move the client from a high intensive level of service to a more independent, recovery focused level of service as appropriate.
  • Utilize evidence based practices in service delivery such as intensive case management, trauma informed care, Motivational Interviewing and Harm Reduction.
  • Support clients who have difficulty with sobriety in making safer choices (ex. Needle exchange).
  • Identify permanent supportive housing units and work closely with individuals to move from street to home as quickly as possible using a Housing First approach.

 
Documentation
  • Maintain documentation standards as set forth by the program contract and PATH policies.
  • Complete progress notes on every face to face/ telephone contact with client or collateral contact by the next business day at 5pm.
 

Qualifications:

  • Maintain and execute confidential information according to HIPAA standards.
  • Experience in homeless services or related social services field with strong skills in case conferencing, program development and implementation and development of policies and procedures
  • Familiarity and comfort working with issues related to homelessness, substance abuse, mental health diagnosis, and history of trauma
  • Experience in the following areas: Psychosocial assessment, Working with clients with co- occurring or tri-morbid conditions (mental health, physical health and substance use), Field- based case management interventions, Housing First and Critical Time Intervention practices, Harm Reduction and Motivational Interviewing strategies, Crisis intervention
  • Flexible work schedule to include some evenings and/or weekends as needed.
  • Ability to provide after hours telephone coverage on a rotating basis as needed.
  • Must possess a high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.
  • Ability to work independently and within a team environment and exercise mature judgment.
  • Ability to provide community and in-home based services on a regular basis.
  • Current Basic Life Support (BLS) certification.
  • Strong written and verbal communication skills.
  • Highly motivated self- starter with the ability to coordinate multiple projects/ tasks simultaneously in a high pressure environment.
  

Required Qualifications

  • Master’s in Social Work (MSW) or Marriage Family Therapist (MFT) with at least two years of experience providing case management services in a non-profit, for profit, government or public service environment.
  • Licensed with the California Board of Behavioral Science
  • Employment Eligibility Verification

  • Successful completion of background screening
  • CPR/First Aid certification
  • Driving is an essential function of this position
    • Must have Valid CA Driver's License
    • Must provide proof of insurance coverage
    • Must be able to qualify for PATH insurance coverage


Work Environment
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
