PATH (People Assisting The Homeless) is seeking candidates with case management experience to join our C3 and C3V program teams in San Diego as a SOAR Case Manager. Applicants for this role must either have current SOAR certification or be able and willing to obtain SOAR certification within the first month of hire.

WHAT WE DO
Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 

The Community Care Coordination (C3) and Community Care Coordination for Veterans (C3V) Programs provide comprehensive care coordination, service navigation, and housing assistance individuals who are homeless or at-risk of homelessness, and are either currently incarcerated or have been recently released from custody. SSI/SSDI Outreach, Access, and Recovery (SOAR) is a service model used to assist these individuals in applying and obtaining Social Security Administration (SSA) benefits.

WHAT IS A SOAR CASE MANAGER?
Working alongside the C3 and C3V teams, the SOAR Case Manager is responsible for guiding eligible program participants through the SOAR process to reduce barriers and assist them with obtaining SSI/SSDI benefits. Responsibilities of the SOAR Case Manager include:
  • Coordinating with C3 and C3V teams to identity eligible participants
  • Completing intake interviews to gather information for SSI/SSDI applications
  • Collaborating clients and the C3 and C3V teams to gather information and documentation, such as medical records, to compete applications
  • Coordinating visits to medical doctors, mental health providers, and other specialists to obtain evidence for SSI/SSDI applications
  • Writing SOAR Medical Summary Reports for individual applications
  • Coordinating with SSA, DDS, and community partners on SOAR projects and services to ensure effectiveness for participants

WHAT YOU BRING
We’re looking for candidates that:
  • Have experience with public benefits and are familiar with the SSA’s Listing of Impairments
  • Have cursory knowledge of medical and psychiatric terminology and demonstrate the ability to write using appropriate terminology
  • Maintain and execute confidential information according to HIPAA and all other applicable standards
  • Have experience providing case management or outreach and engagement to vulnerable populations
  • Have training or experience with evidence-based practices such as Motivational Interviewing and Trauma Informed Care
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment

MANDATORY REQUIREMENTS
For this role the SOAR Case Manager must:
  • Hold a Bachelor’s Degree in a related field and have at least 2 years of experience OR an equivalent combination of education and/or experience
  • Be SOAR certified or be able and willing to obtain SOAR certification within 1 month of hire
  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Have reliable transportation and:
    • A valid driver’s license
    • Proof of insurance and ownership for personal vehicles used during work duties
    • The ability to qualify for PATH insurance coverage
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

COVID-19 VACCINATION
Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
