PATH (People Assisting The Homeless) is seeking candidates with social services experience to join our ICMS Program as a Case Manager. This position will be based out of our satellite office in Long Beach and provide field-based services throughout southern LA County.

Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 
PATH’s Long Beach Intensive Case Management Services (ICMS) Program provides services to individuals experiencing homelessness with chronic medical conditions and co-occurring disorders.  Intensive case management services provided by the program are designed to comprehensively address clients' psychosocial service needs with the ultimate goal of helping them attain their housing stability and improve their quality of life. 
As part of the ICMS team, the Case Manager provides field-based, direct client services to program participants. Responsibilities of the Case Manager include:

  • Providing case management services including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, community integration, and linkage to community providers for substance use, primary and mental health care
  • Collaborating with clients to develop individualized treatment plans that address short- and long-term goals
  • Providing on-going case management support to assess plan progress and ensure goals are met or adjusted as necessary
  • Providing strengths-based case management and service coordination designed to assist clients in obtaining and maintaining stable housing
  • Maintain documentation standards as set forth by the program contract and PATH policies

We’re looking for candidates that:
  • Have experience in permanent supportive housing
  • Demonstrate knowledge of and/or experience with a "Whatever It Takes" philosophy that integrates advanced, evidence-based case management techniques such as critical time intervention, trauma informed care, harm reduction, non-violent crisis intervention, and motivational interviewing
  • Maintain and execute confidential information according to HIPAA standards
  • Demonstrate knowledge of or experience with evidence-based case management techniques including critical time intervention, trauma informed care, harm reduction strategies, crisis intervention techniques and motivational interviewing
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment

For this role the Case Manager must:
  • Have a minimum of 2 years of experience OR hold a Bachelor's Degree with 1 year of experience working in homeless services, mental health, substance use, or co-occurring disorders or with vulnerable populations providing intensive case management services 
  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Have reliable transportation and:
    • A valid driver’s license
    • Auto Insurance
    • The ability to qualify for PATH insurance coverage
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test
