PATH (People Assisting The Homeless) is seeking qualified social services candidates with case management experience to join out SSVF Program in San Diego as a Case Manager.

Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 
The Supportive Services for Veteran Families (SSVF) is a rapid rehousing and prevention services program that serves Veterans experiencing homelessness or at-risk of homelessness. The program offers highly individualized services in the form of case management, housing location, and temporary financial assistance. The goal of the program is to rapidly house program participants and provide support towards long-term housing stability.
As part of the SSVF team, the Case Manager provides supportive services to participating veterans to help them break the cycle of homelessness and attain housing stability. Responsibilities of the Case Manager include:
  • Conducting intake and needs assessments
  • Collaborating with clients and other SSVF staff to develop Individualized Service Plans (ISPs)
  • Providing ongoing case management services to support ISP goal completion including, but not limited to:
    • Assistance with benefits establishment
    • Assistance with obtaining identification documents
    • Employment linkage and assistance
    • Assistance with housing navigation and application
    • Providing referrals and linkages to substance abuse, primary, and mental health care services
  • Providing a high-quality customer service environment for all clients and guests
  • Maintaining documentation standards as set forth by the program contract and PATH
We’re looking for candidates that:
  • Hold a Master’s Degree in a human services related field (e.g., MSW, MFT)
  • Demonstrate strong skills with computers including proficiency with Microsoft Office Suite
  • Have experience in case management with highly vulnerably populations, particularly those with health, mental health, and substance abuse issues.
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
For this role the Case Manager must:
  • Hold a Bachelor’s Degree in a human services related field
  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Have reliable transportation and:
    • A valid driver’s license
    • Proof of insurance and ownership for personal vehicles used during work duties
    • The ability to qualify for PATH insurance coverage
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of vaccination as part of the pre-employment credentialing process.
