PATH is a $100M+ non-profit provider of shelter and supportive services for the homeless with locations throughout Southern California, Santa Barbara, and San Jose, primarily serving veterans, families, and the chronically homeless. Funding comes from government agencies, foundations, and individuals, and we currently have over 100 grants from over 30 funders. PATH founded PATH Ventures (PV) in 2007, a non-profit developer of permanent supportive housing. With projects located throughout Southern California, PV possesses an ownership interest via limited partnerships in over a dozen residential communities plus several projects in active predevelopment.  

PATH’s Permanent Supportive Services (PSS) programs provide Intensive Case Management Services (ICMS) in a Permanent Supportive Housing (PSH) setting to individuals who have experienced homelessness and have a history of chronic health, mental health, and/or co-occurring conditions. Services are centralized and provided to tenants living on-site to ensure ease of access to the resources necessary for long-term housing stabilization, improving their quality of life and community integration. 


This position will be a hybrid PSH and Critical Time Intervention (CTI) case manager. PSH Case Managers work collaboratively with a range of community service providers and other PATH staff members to coordinate services and ensure tenants living in PSH are progressing towards their individual goals and the goals of the program. They support tenant success by offering recovery orientated, evidenced-based case management services on-site, in tenants’ homes, and the community. Case Managers working in the PSH programs are also responsible for tenant application, including eligibility screening, assistance with compiling required documentation, and developing support plans that ensure a smooth transition from the streets to permanent housing. This position will be based out of the PATH Metro Villas I site and will provide services both in tenants homes and the community.  

The Critical Time Intervention (CTI) Case Manager will work with homeless and chronically homeless individuals and families with histories of mental health and substance use issues as they transition from experiencing homelessness to living in a permanent supportive housing community. The CTI case manager will provide recovery-oriented services, integrating evidenced based practice effective with a diverse population with co-occurring disorders in a phased-out approach during their first 9 months of transitioning into permanent housing.

  • Meet with tenants on a minimum of a weekly basis to provide strengths-based case management and service coordination services designed to assist tenants in obtaining and maintaining stable housing.  

  • Perform initial comprehensive bio psychosocial assessment with quarterly updates.  

  • In collaboration with the tenant, create a person-centered goal plan addressing short- and long-term goals to be reviewed and completed every 90 days.  

  • Develop housing retention plans that include 3 concrete interventions to support housing maintenance as evidenced by written housing plan support plan.  

  • Provide in home and mobile case management services in the areas of, but not limited to: independent living skills, housing stabilization, money management, community integration, employment, benefits establishment, linkage to community providers for problematic substance use, primary and mental health care, and all other services needed to assist tenants in reaching their treatment plan goals.  

  • Conduct crisis and risk assessments in consultation with supervisor and/or team.  

  • Provide crisis intervention services focused on enhancing the tenants’ ability to independently problem solve, utilize effective coping skills, and manage and self -coordinate own care.  

  • Utilize evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention and Housing First practices.  

  • Mediate and advocate on behalf of the tenant to help him/her obtain and keep housing, health care benefits, entitlements, transportation, etc.  

  • Promote tenant health and wellness through coordination of services with local healthcare providers including, but not limited to, medical health, mental health, cognitive health, substance dependency, and trauma-services.  

  • Provide case management coverage on weekends, evenings, and holidays as needed.  

  • Create a monthly calendar of activities including programing at least 2x/week including at least 1 community integration outing, 1 guest speaker, and 1 skill building class (healthy living, recovery, social enterprise, etc.)  

  • Maintain documentation standards as set forth by the program contract and PATH policies.  

  • Complete progress notes on every face to face/ telephone contact with tenant or collateral contact within 48 hours.  

  • Ensure tenant files are up to date with all file paperwork including consents, ISP’s, homeless/disability verifications, and overall general documentation  

  • Submit all reports on or before deadlines (Data Integrity Reports, narrative reports, etc.)  

  • Input tenant data into online documentation systems within 48 hours. 

  • CTI : Work in collaboration with on-site case management team and tenant to develop a housing stabilization plan in collaboration addressing short term and long-term goals. Provide on-going case management support to assess progress and ensure treatment plan outcomes are met or changed as needed.  

  • Provide intentional case management supports that move the client from a highly intensive level of service to a more independent, recovery focused level of service as appropriate. 


  • A high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.  

  • Ability to work independently and within a team environment and exercise mature judgment.  

  • Ability to provide community and in-home based services on a regular basis.  

  • Strong written and verbal communication skills. 

  • High level of motivation and self-starting nature with the ability to coordinate multiple projects/tasks simultaneously in a fast-paced environment.  

  • Ability to maintain and execute confidential information according to HIPAA standards. 


  • Spanish language proficiency in addition to the primary language used in the workplace. 


  • Eight (8) years of professional experience OR equivalent combination of education and experience including:  

  • Bachelor’s Degree in a related field OR comparable years of professional experience.  

One (1) year of experience in homeless services, case management, health services,  mental health, substance use disorders, and/or permanent supportive housing.  

  • Experience using data management systems to document services.
  • ​Experience using Microsoft tools, including Excel and Word. 

For this role, a successful candidate must: 

  • Demonstrated computer proficiency, particularly with Microsoft Office (Word, Excel) and electronic health record systems. Intermediate to advanced Excel skill level preferred.  
  • Training in Harm Reduction, Motivational Interviewing, Trauma Informed Care, and other related trainings are preferred, but not required. 
  • Have employment eligibility verification. 
  • Have or be able and willing to obtain CPR/First Aid training 
  • Provide proof of full COVID-19 vaccination
  • Have reliable transportation and: 
    • A valid driver’s license.  
    • Proof of insurance and ownership for personal vehicles used during work duties. 
    • The ability to qualify for PATH insurance coverage.  
  • ​Successfully complete the following as a condition of hire:

    • Drug Test
