PATH (People Assisting The Homeless) is seeking candidates with highly advanced case management skills to join our housing partnership program as a Residential Service Coordinator.

WHAT WE DO
Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 

WHAT IS AN RESIDENTIAL SERVICE COORDINATOR?
The Residential Service Coordinator (RSC) works as part of the case management team to provide supportive services to RPSS tenants and assist them in breaking the cycle of Residential Service Coordinator - RPSS PATH is an Equal Opportunity Employer Page 2 of 5 Grant 490 April 2021 homelessness. As part of this team, the RSC supports residents’ transition from street to housing by identifying their individual needs and utilizing a coordinated system to facilitate and ensure their ability to access and maintain identified necessary services. Focusing on housing and health as necessary outcomes for success, the RSC works with community partners to address barriers, assist with maintaining and sustaining housing, and supporting health needs. Responsibilities of the Residential Services include:
 
• Coordinate move-in into RPSS housing units.
• Liaison between tenant and community based case management team
• Ensure tenant well-being and unit habitability through a regular schedule of unit inspections and wellness checks.
• Provide assistance for critical issues that impact housing stability
• Respond to housing and tenant-related emergencies during normal business and after hours.
• Identify and secure community resources in coordination with assigned case manager
• Gain expert knowledge of homeless populations, common barriers to housing, and over all knowledge of community resources
• Understand the building lease term agreement, and provide education to tenants and case managers on lease requirements
 • Maintain client related data tracking systems, including case notes and complete HMIS and Salesforce entries
• Prepare case-related reports including but not limited to: outcomes, successes and challenges
• Generate client data for reporting and understand the overall importance of data collection
• Maintain complete and accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and PATH guidelines
• Complete follow-up and retention services, as necessary, and provide back-up documentation in client file.
• Participate in staff meetings and training as assigned by your supervisor.
 
 
WHAT YOU BRING
  •  2 years of experience in a human services or behavior related field
  • Demonstrated ability to utilize computers and proficiency in Microsoft Office.
  • A professional demeanor and strong interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong written and verbal communication skills.
  • Demonstrated ability to maintain and execute confidential information according to HIPAA standards
  • High level of motivation self-starting demeanor with the ability to coordinate multiple projects/tasks simultaneously in a high-pressure environment.
  • Ability to work with diverse communities, good problem solving and conflict resolution skills.
  • Training in Motivational Interviewing, Trauma Informed Care, and other related trainings are preferred, but not required.
MANDATORY REQUIREMENTS

  • Successful completion of background screening & drug test
  • Ability to work flexible hours which may include evenings or weekends
  • Ability to obtain CPR/First Aid certification through PATH
  • Provide proof of full COVID-19 vaccination
  • Driving is an essential function of this position. To meet the requirements of for this position, the incumbent:
    • Must hold a Valid CA Driver's License
    • Must possess their own vehicle and provide proof of ownership
    • Must provide proof of insurance coverage
    • Must be able to qualify for PATH insurance coverage

Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of vaccination as part of the pre-employment credentialing process.
