PATH (People Assisting The Homeless) is seeking experience social services professionals to join our PSS team a CTI Case Manager.

Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 
PATH’s Permanent Supportive Services (PSS) programs provide person-centered, culturally competent supportive services to Permanent Supportive Housing (PSH) residents who have experienced homelessness and have a history of chronic health, mental health, and/or co-occurring conditions. The Critical Time Intervention (CTI) teams at these sites provide recovery-oriented services that integrate evidence-based practices in a phased-out approach during residents’ first 9 months of transitioning into permanent housing.
As part of the PSS team, the CTI Case Manager provides case management services to clients during their transition to housing to ensure their success in stabilizing in housing, improving the quality of their lives, and integrating into the community. Responsibilities of the CTI Case Manager include:
  • Utilizing the CTI model and phased approach to provide intentional case management supports that move clients from a high-intensive level of service to a more independent, recovery-focused level of service over time
  • Conducting initial comprehensive biopsychosocial assessments to identify needs and assist with the development of intervention plans
  • Collaborating with clients and other staff members to develop housing stabilization and intervention plans that identify barriers and address short- and long-term goals
  • Providing on-going case management support for all identified needs necessary for housing stabilization including substance abuse treatment, primary and mental health services, and coordination of community services
  • Supporting lease-up activities for PSH sites including conducting eligibility screenings and assisting with documentation
We’re looking for candidates that:
  • Execute and maintain confidential information according to HIPAA standards
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
For this role the CTI Case Manager must:
  • Have at least 8 years of professional experience OR an equivalent combination of education and experience including:
    • A Bachelor’s Degree in a related field OR comparable years of professional experience
    • At least 1 year of experience in homeless services, case management, health services, mental health, substance use disorders, and/or permanent supportive housing
  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Have reliable transportation and:
    • A valid driver’s license
    • Proof of insurance and ownership for personal vehicles used during work duties
    • The ability to qualify for PATH insurance coverage
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test
