PATH (People Assisting The Homeless) is urgently seeking candidates with experience in homeless services or serving vulnerable populations to join our Housing Partnerships Program as LeaseUp Coordinator. This role will be based out of our regional headquarters in Los Angeles and support LA County's Recovery Rehousing Program in the Metro LA area.

WHAT WE DO
Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.
PATH’s Housing Partnerships Program (HPP) provides landlord outreach and engagement services to develop a full range of permanent housing opportunities for individuals and families experiencing homelessness. The Recovery Rehousing program is part of LA County’s COVID-19 response and focuses on assisting those experiencing homelessness who are at a high risk for COVID-19 or have a high level of acuity and need in obtaining permanent housing.
WHAT IS A LEASEUP COORDINATOR?
As part of the HPP team, the LeaseUp Coordinator plays a key role in connecting participants in the Recovery Rehousing program with sustainable housing opportunities by facilitating coordination between the LeaseUp LA Program, PATH, and community partners. Responsibilities of the LeaseUp Coordinator include:
WHAT YOU BRING
- Conducting daily outreach to assigned community partners to develop relationships with case managers and navigators to assist them with finding a home for their clients
- Attending and representing the LeaseUp program at assigned Housing Surge calls with Recovery Rehousing providers
- Attending and representing the LeaseUp program at assigned Coordinated Entry System (CES) meetings
- Identifying appropriate housing opportunities for participants based on their needs and available housing in the LeaseUp portfolio and facilitating the matching process with case managers
- Training case managers and navigators on use of the LeaseUp website
- Assisting with applications for housing opportunities and ensuring a timely follow-up for responses from the HPP team
- Maintaining a high-level of responsiveness to inquiries regarding housing opportunities
- Ensuring requests for support from community partners receive responses and appropriate follow-up information within 24 hours
We’re looking for candidates that:
- Demonstrate knowledge and experience with advanced and evidence-based interventions including:
- Engagement and rapport building with hard-to-reach clients
- Harm Reduction and Motivational Interviewing strategies
- Housing First
- Non-violent crisis intervention techniques
- Case management interventions proven effective with vulnerable populations
- Maintain and execute confidential information according to HIPAA standards
- Demonstrate strong problem solving and conflict resolution skills
- Effectively use technology as a work tool including demonstrated proficiency with Microsoft Office Suite and database software
- Work just as well independently as they do on a team
- Exercise mature judgement, and are highly motivated, self-starting and proactive
- Are excellent at communicating, whether in writing or verbally
- Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
For this role the LeaseUp Coordinator must:
- Hold a Bachelor’s Degree in a related field OR have at least 2 years of relevant experience working in homeless services, mental health, substance use and co-occurring disorders, or with vulnerable populations providing intensive case management services
- Be able and willing to work flexible hours which may include evenings or weekends
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Provide proof of full COVID-19 vaccination
- Have reliable transportation and:
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for PATH insurance coverage
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
PATH will require all employees to be fully vaccinated
for COVID-19. All prospective hires will be expected to provide proof of vaccination as part of the pre-employment credentialing process.
