PATH (People Assisting The Homeless) is seeking candidates with experience in social services or working with vulnerable populations to join our CBEST ICMS Program in south Los Angeles County as a Case Manager. This position is co-located at the DPSS office, but primary operates in the field providing services in clients’ homes, shelters, on the streets, or wherever they are currently residing.

WHAT WE DO
Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 
 
The Countywide Benefits Entitlement Services Team (CBEST) Intensive Case Management Services (ICMS) program provides benefits advocacy for individuals who are currently homeless or at imminent risk of homelessness and are eligible for disability benefits. These services are focused on maximizing program participants’ income, leading to increasing access to permanent housing and leading to long-term housing stability.
 
WHAT IS A CASE MANAGER?
As part of the multidisciplinary CBEST ICMS team, Case Managers employ a “whatever it takes” approach and work closely with the LA DPSS and community partners to identify, engage, and provide field-based supportive services to eligible participants. Responsibilities of the Case Manager include:
  • Completing client intakes and vulnerability assessments for all incoming clients
  • Collaborating with clients and other program staff to individualized case plans
  • Assist clients with addressing their immediate needs such as access to temporary housing, food, and clothing
  • Connecting, linking, and providing clients with appropriate resources and partner referrals to ensure access to necessary services including:
    • DPSS eligibility workers for assistance enrolling in services such as Medi-Cal, GR, or CalFresh
    • The homeless system of care through entry into HMIS and CES completion
    • Arranging or directly providing transportation to appointments and accompanying clients to appointments as necessary
    • Assisting clients with engaging with health and mental health services
    • Providing housing aid including assistance with documentation  
    • Direct financial assistance to obtain necessary goods or services
    • Referrals to legal services
  • Collaborating with care providers to monitor and follow up on client progress to ensure clients receive necessary supports and services in a timely manner
  • Monitoring and following up with clients on their progress to advance them to the next step in CBEST once they have achieved their case plan recommendations
 
WHAT YOU BRING
We’re looking for candidates that:
  • Have case management experience
  • Are familiar and have experience with CHAMP and HMIS
  • Demonstrate knowledge of or experience with evidence-based case management techniques including critical time intervention, harm reduction strategies, crisis intervention techniques, and motivational interviewing
  • Maintain and execute confidential information according to HIPAA standards
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
 
MANDATORY REQUIREMENTS
For this role the Case Manager must:
  • Have a least 1 year of experience working in homeless services, mental health, substance use and co-occurring disorders, or with vulnerable populations
  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Have reliable transportation and:
    • A valid driver’s license
    • Proof of insurance and ownership for personal vehicles used during work duties
    • The ability to qualify for PATH insurance coverage
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test
