Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 
Our Interim Housing programs provide low-barrier, high-tolerance settings for program participants, referred to as guests, who are experiencing homelessness and have a variety of complex health, functional and social needs. These home-like environments are coupled with support and case management services to assist guests with developing a plan to break the cycle of homelessness, stabilize in housing and reintegrate into the community.
The Residential Service Coordinator works as part of a team of case managers providing supportive services to formerly homeless Veterans at the Zephyr Apartments. This location is a permanent supportive housing site with onsite services for Zephyr Apartment residents. The Residential Service Coordinator (RSC) will assist clients in breaking the cycle of homelessness by transitioning clients from street to housing and accessing and maintaining necessary services among health care and social services through a coordinated system. RSC will provide individualized supportive services plan by helping each client develop a plan to address barriers, support health needs, increase income, and maintain and sustain housing. Responsibilities of the Residential Coordinator include:
  • Maintain phone line to respond to bed availability requests.
  • Conduct intake and individualized needs assessment for all clients and collaborate with clients to develop Individualized Service Plan (ISP) that address maintaining stable housing,
  • maintaining/increasing income and other personals goal identified by the client.
  • Meet with clients on a regular and/or as needed basis to review, evaluate and support completion of ISP goals/plans.
  • Provide ongoing case management support as needed with benefits establishment (GR, SSI, Veterans pension, etc..), assistance obtaining ID, birth certificate, social security cards, provide employment linkages and assistance, housing referrals as needed and any health related referrals as identified on the ISP.
  • Provide assistance with transportation for critical appointments that impact housing stability.
  • Participate in staff meetings and training as assigned by your supervisor.
  • Identify and secure community resources.
  • Gain expert knowledge of homeless populations, common barriers to housing, and over all knowledge of community resources.
  • Maintain client related data tracking systems, including case notes and complete Service Point-HMIS entries.
  • Prepare case-related reports including but not limited to: outcomes, successes and challenges
  • Generate client data for reporting and understand the overall importance of data collection.
  • Maintain complete and accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and PATH guidelines
  • Complete follow-up and retention services, as necessary, and provide back-up documentation in client file. 
We’re looking for candidates that:
  • Possess a minimum of a Bachelor’s Degree in human services/behavior, a Master’s Degree is preferred.
  • Demonstrated ability to utilize computers and proficiency in Microsoft Office.
  • Projects a professional demeanor and interpersonal skills.
  • Able to work independently and as part of a team.
  • Strong written and verbal communication skills.
  • Must maintain and execute confidential information.
  • A highly motivated self-starter and ability to coordinate multiple projects/tasks simultaneously in a high pressure environment.
  • Ability to work with diverse communities, good problem solving and conflict resolution skills. 4
  • Training in Motivational Interviewing, Trauma Informed Care, and other related trainings are preferred, but not required.
For this role the Residential Coordinator must:
  • Commit to maintaining regular and consistent attendance
  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of COVID-19 vaccination
  • Have reliable transportation and:
    • A valid driver’s license
    • Auto Insurance
    • The ability to qualify for PATH insurance coverage
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test
