PATH (People Assisting The Homeless) is seeking candidates with Master’s level credentials (MSW or MFT) and experience in homeless or permanent supportive services to join our new PSH site in Mission Valley as Clinical Case Manager.

WHAT WE DO
Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 
 
Mission Valley is a new Permanent Supportive Housing (PSH) development in San Diego that will provide homes and on-site supportive services for people who have experienced homelessness. The PSH setting centralizes services on-site to ensure residents have immediate and easy access to the resources necessary for long-term housing stabilization, improving their quality of life and community integration.
 
WHAT IS A CLINCAL CASE MANAGER?
As part of the multidisciplinary PSH team, the Clinical Case Manager provides residents with individualized clinical support to assist them in improving their wellness and housing stability. Responsibilities of the Clinical Case Manager include:
  • Conducting comprehensive bio-psychosocial assessments and coordinating with the PSH team to develop a plan of intervention that addresses housing stability including necessary case management support, substance use treatment, mental health services and coordination of needed community services
  • Collaborating with clients to develop individualized care plans that address short- and long-term goals
  • Providing on-going case management support and individual therapy to assess progress and ensure treatment plan outcomes are met or adjusted as necessary
  • Conducting crisis and suicide risk assessments followed by the provision of appropriate intervention services that work to enhance residents’ ability to independently problem solve, utilize effective coping skills and manage and self-coordinate own care
  • Maintaining documentation, reporting, and program data in accordance with PATH policies and program contract
 
WHAT YOU BRING
We’re looking for candidates that:
  • Maintain and execute confidential information according to HIPAA standards
  • Have experience in homeless services or related social services field, demonstrating strong skills in case conferencing and housing retention services with a holistic approach
  • Are familiar and comfortable working with issues related to homelessness, substance use, mental health diagnosis and history of trauma
  • Have experience with psychosocial assessment, working with clients with co-occurring or tri-morbid conditions, field-based case management interventions, Housing First and Critical Time intervention practices, Harm Reduction and Motivational Interviewing strategies and Crisis Intervention
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
 
MANDATORY REQUIREMENTS
For this role the Clinical Case Manager must:
  • Hold a Master’s Degree in Social Work (MSW) or Marriage & Family Therapy (MFT) and have at least 2 years of experience providing case management services
  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Have reliable transportation and:
    • A valid driver’s license
    • Auto Insurance
    • The ability to qualify for PATH insurance coverage
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test
