PATH (People Assisting The Homeless) is seeking candidates with proven experience working within a Harm Reduction framework to join our new Mission Valley PSH site as a Harm Reduction Specialist.

WHAT WE DO
Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 
 
Mission Valley is a new Permanent Supportive Housing (PSH) development in San Diego that will provide homes and on-site supportive services for people who have experienced homelessness. The PSH setting centralizes services on-site to ensure residents have immediate and easy access to the resources necessary for long-term housing stabilization, improving their quality of life and community integration.
 
WHAT IS A HARM REDUCTION SPECIALIST?
As part of the multidisciplinary PSH team, the Harm Reduction Specialist provides expertise and supportive services related to substance use. Services provided integrate evidence-based practices and use a harm reduction model to to assist individuals with long-term housing stability. Responsibilities of the Harm Reduction Specialist include:
  • Performing initial comprehensive substance use assessments and developing treatment plans for residents that address needs for on-going case management services, substance use treatment, mental health services and coordination of community provided services
  • Providing on-going support to assess treatment plan progress and ensure outcomes are met or changed as necessary
  • Meeting with residents at least twice monthly to provide therapeutic services that help residents maintain housing and support case management plans by addressing functional and substance use barriers
  • Ensuring residents are connected and have access to necessary supportive services identified in treatment plan by making referrals and accompanying clients to appointments as requested
  • Providing crisis intervention and case management services designed to enhance residents’ independence, including their ability to problem-solve, utilize effective coping skills, and manage and coordinate own care, in effort to move them from high-intensity to a recovery focused level of services 
  • Maintaining documentation standards as set forth by PATH policies and the program contract
 
WHAT YOU BRING
We’re looking for candidates that:
  • Have experience in homeless services, case management, mental health and supportive housing
  • Maintain and execute confidential information according to HIPAA standards
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
 
MANDATORY REQUIREMENTS
For this role the Harm Reduction Specialist must:
  • Have a background with proven experience working within a Harm Reduction framework to assist individuals struggling with substance use challenges including:
    • A Master’s Degree with at least 2 years of experience providing substance use case management services
    • A Bachelor’s Degree and Harm Reduction or SUD Specialist credentials
    • An Associate’s Degree with SUD credentialing
    • Individuals with lived experience and SUD credentialing
  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Have reliable transportation and:
    • A valid driver’s license
    • Auto Insurance
    • The ability to qualify for PATH insurance coverage
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test
