PATH (People Assisting The Homeless) is seeking Master's Level (MSW or MFT) candidates to join our Veteran Services – VASH Program as a Case Manager. This is a full-time position that will work mostly in the field and be based out of our North Long Beach location.

WHAT WE DO
Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 
 
Our Veteran Affairs Supportive Housing (VASH) Program works to promote housing stability among very low-income Veteran individuals or families who are experiencing or at imminent risk of homelessness.
 
WHAT IS A CASE MANAGER?
Case Managers work alongside a team of other case managers, housing locators, and community provides to provide field-based, direct client services to veterans experiencing homelessness. Responsibilities of the Case Manager include:
  • Providing field-based, mobile case management services including, but not limited to, independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment and linkage to community providers for substance abuse and primary and mental health care.
  • Providing on-going strengths-based case management and service coordination services designed to assist clients in obtaining and maintaining stable housing
  • Collaborating with clients to develop individualized treatment plans that address short- and long-term
  • Providing crisis intervention services focused on enhancing clients’ ability to independently problem solve, utilize effective coping skills, and manage and self-coordinate care
  • Utilizing evidenced-based practices in service delivery such as intensive case management, motivational interviewing, harm reduction, critical time intervention, and housing first practices
WHAT YOU BRING
We’re looking for candidates that:
  • Maintain and execute confidential information according to HIPAA standards
  • Demonstrated knowledge or experience with case management techniques including critical time intervention, harm reduction strategies, crisis intervention techniques and motivational interviewing
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
MANDATORY REQUIREMENTS
For this role the Level I Case Manager must:
  • Hold a Master's Degree in a related field; MSW strongly preferred
  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Have reliable transportation and:
    • A valid driver’s license
    • Auto Insurance
    • The ability to qualify for PATH insurance coverage
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test
