PATH (People Assisting The Homeless) is seeking candidates passionate about making a positive impact in the San Diego community by joining our Whole Person Wellness (WPW) Program as a Peer Support Specialist for the remainder of the program scheduled to end 12/31/2020. Candidates with lived experience in homelessness, mental health or substance use challenges are encouraged to apply!

WHAT WE DO
Since its foundation in 1984, PATH has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.
The WPW program provides comprehensive care coordination services to high-utilizers of health care services. WPW services are facilitated buy a multidisciplinary Service Integration Team (SIT) that includes clinicians, peer support specialists, case managers, and housing specialists. SIT’s provide a holistic approach to linking program participants to services that will improve their wellness and housing stability through street outreach, system navigation, and housing navigation services.
WHAT IS A PEER SUPPORT SPECIALIST?
As part of the SIT, Peer Support Specialists identify and build positive relationships with homeless individuals and families living on the streets or in vehicles. Peer Support Specialists assist clients in breaking the cycle of homeless by:
WHAT YOU BRING
- Collaborating with the SIT and clients to develop Individual Service Plans (ISP) that identify clear objectives related to stabilizing in housing
- Providing case management such as assisting with housing location, temporary financial assistance, and assistance with enrolling in MediCal and accessing health services
- Providing crisis intervention services to the homeless community
- Monitoring client progression through their ISP
- Assisting clients with obtaining documentation necessary to obtain housing
We’re looking for candidates that:
- Have experience providing direct client services to vulnerable individuals
- Demonstrate ability to work with diverse communities
- Work just as well independently as they do on a team
- Exercise mature judgement, and are highly motivated, self-starting and proactive
- Are excellent at communicating, whether in writing or verbally
For this role the Peer Support Specialist must:
READY TO MAKE A DIFFERENCE THROUGH ACTION?
- Have lived experience with homelessness, mental health, or substance use challenges; Peer Support Certification is strongly preferred
- Be able and willing to work flexible hours which may include evenings or weekends
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Have reliable transportation and:
- A valid driver’s license
- Auto Insurance
- The ability to qualify for PATH insurance coverage
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
