Starting $19.25/hr (Base pay) and eligible for a Premium Pay Differential (50% Added to base hourly rate)

PATH is a $75M+ non-profit provider of services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing. 

What is a Client Support Specialist?
Working alongside Project Room Key’s Program Manager, other Client Support Specialists, clinical medical staff, LAPD, LAFD, paramedics, and other first responders, the Client Support Specialist is an integral component of Los Angeles County’s emergency response to COVID-19.

The Client Support specialist provides case management services to individuals who have been referred through the Los Angeles Homeless Services Authority (LAHSA), specifically those who are experiencing homelessness, have a variety of complex health, functional, and social needs, and are at risk of developing complications if exposed to COVID-19. Working onsite at a motel designated by LAHSA, the Client Support Specialist will deliver food and supplies, support clients via telephone and in person, and work with the on-site team and LAHSA to discharge individuals once they have found a permanent home or upon the end of the program.

Make a Difference through Action
Responsibilities in this role will include, but are not limited to:
  • Collaborating with LAHSA to process client intake and services, including:
    • Assisting with intake and room assignments
    • Providing brief orientations to new clients
    • Coordinating care
  • Maintaining contact with assigned clients by ensuring client phone access and contacting clients by phone at minimum 2x/daily to check in on client’s needs and provide essential items
  • Logging all client interactions and supplies provided
  • Coordinating and supporting client through the discharge process, including handling discharge planning
  • Providing education and information to clients about COVID-19 and how to manage any unpleasant emotions they are experiencing due to being isolated
  • Providing meals to clients and coordinating with the Program Manager to assist with ensuring seamless meal delivery
  • Triaging for appropriate intervention (nursing, mental health, supplies, food, etc.) as necessary and supporting team of staff with utilizing de-escalation skills in the event a client becomes escalated
  • Maintaining documentation standards as set forth by PATH policies and  the specific program, including maintaining compliance with HIPAA policies and practices, updating the bed management information, and ensuring intake documentation
What You Bring
  • Proven ability to maintain and execute confidential information according to HIPAA standards
  • Demonstrated knowledge and or experience with case management techniques, including critical time intervention, harm reduction strategies, crisis intervention techniques and motivational interviewing
  • Flexibility work schedule to include some evenings and/or weekends as needed
    • Overnight and Weekend Shifts Required - Ranging from 8-12 hours, 3-5 Days per week
  • HMIS experience preferred
  • A high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs
  • Ability to work independently and within a team environment and exercise mature judgment
  • Ability to provide community and in-home based services on a regular basis
  • Strong written and verbal communication skills
  • Motivated, self-starting orientation with the ability to coordinate multiple projects/tasks simultaneously in a high-pressure environment

  • Ability to work flexible hours which may include alternative shifts, evenings, holidays or weekends
  • 8 or more years of experience in homeless services or related field
