The position will work to coordinate with the Lead Outreach Coordinator & Program Manger to expand and coordinate full geographic SPA coverage of homeless outreach services from all providers. The position will be responsible to work with community organizations in establishing outreach best practices and develop tools to assess the effectiveness of the community’s outreach efforts. The position will also support in the coordination of the SPA wide centralized outreach request system, once it is available.

- Work with CES Coordinators to develop strategies to increase awareness and implementation of CES among homeless service providers and the community. Respond within the agreed time for outreach requests for outreach services and send follow-up information to requestor and supervisor. Represent PATH at community and provider meetings. Engage and assess capabilities of community groups, churches and other non-professional outreach teams. Work with CoC, City, County, State and Federally funded street outreach teams as well as first responders to establish best possible practices in identifying and working with individuals who has a long history of not being connected to community resources. Track outreach response times and outcomes of centralized requests. Participate in staff meetings and training as assigned by your supervisor.
- Identify potentially underserved geographic areas/ locations and work with outreach teams to engage and assess homeless individuals throughout the entire SPA. Actively work with outreach teams and LAHSA to identify hotspots and encampments to target for coordinated outreach. Maintain a SPA level outreach team inventory to document what teams provide what services in the entire SPA. Coordinate with the Associate Director and Lead Outreach Coordinator to ensure adequate geographic outreach coverage, in conjunction with the new E6 Multidisciplinary Teams.
- In collaboration with the Lead Outreach Coordinator & Program Manager provide support in capacity building of outreach teams through trainings or other learning opportunities. Facilitate outreach meetings with outreach teams within the SPA. Promote the use of evidence based practices effective with the homeless population with outreach teams within the SPA.
- Maintain documentation standards as set forth by the program contract and adhere to all PATH policies. Optimize response to referrals for most appropriate and consistent outreach contacts. Complete paperwork on each face-to-face and telephone contact with clients, or with any collateral contact, by the close-of-business on the next working fay. Ensure all program data is accurate and entered into the appropriate program documentation system as required. Prepare case related reports including, but not limited to, mental health evaluations, outcome and success reporting and disability reports. Generate client and community resource data for reporting. Maintain complete and accurate documentation of service objectives and outcomes, as well as other requirements as outlines by Federal, State, County, and PATH guidelines.
- Experience in street outreach with highly vulnerable populations, particularly those with health, mental health, and substance abuse issues.
- Demonstrated knowledge and experience with advanced street outreach techniques and evidence-based interventions:
- Engagement and rapport building with hard to reach clients
- Harm Reduction and motivational interviewing strategies
- Housing First
- Non-violent crisis intervention techniques
- Case management intervention that has been proven effective in vulnerable populations
- Able to work independently and as part the team
- Ability to exercise mature and professional judgment.
- Strong written and verbal communication skills.
- Good problem solving and conflict resolution skills.
- A highly motivated self-starter and ability to coordinate multiple projects/tasks simultaneously.
- Knowledge of maintaining and executing confidential information using HIPAA standards.
- Computer skills with proficiency in Microsoft Office software.
- Flexible, adaptable and have the capability to work in a fast paced, professional environment.
- Availability to work evening and weekends as requested by management and based on the operational needs of the program
- Bachelor’s Degree in a related field and/or 2 years of experience in street outreach with highly vulnerable populations, particularly those with health, mental health, and substance abuse issues
- Ability to maintain regular, consistent attendance
- Employment Eligibility Verification.
- Reliable transportation and a valid California Driver’s License
- Ongoing, active automobile insurance that complies with state requirements
- Updated tuberculosis test.
- Successful completion of background screening.
- CPR/First Aid Training.
- Driving is an essential function of this position

- Must be able to qualify for PATH insurance coverage
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
