The Direct Care Monitor will provide care and assistance to the residents of the facility. The Direct Care Monitor works as part of a team with residents and their care providers (case managers). Some outcomes may include: benefit enrollment and employment opportunities to increase income; linkages to community resources to decrease social isolation; and, focusing on behavioral health and substance abuse needs. This position is primarily responsible for overseeing the safety and basic needs of the residents. The Direct Care Monitor follows established low barrier guidelines to create a safe and stable environment for residents entering the program. This position may also be responsible for assisting with the duties that include meal preparation, answering phones and assisting with general facility needs as they arise.

- Complete bed count list before the end of each shift and review with staff for next shift
- Participate and be present for triage sessions
- Conduct at least 9 rounds per shift and complete the necessary documentation of observations during each round
- Compose triage reports on the behaviors and needs of each resident
- Monitor chore assignment and completion, informing providers of any resistance or lack of participation
- Recommend residents for thanks, “kudos” who go above and beyond to improve the community
- Monitor resident messages and mail
- Inventory supplies and set up for next shift
- Assist in cleaning and keeping the facility organized
- Monitor basic hygiene with clients
- Answer phones and take messages as needed
- If assigned to Cottage Health contacts, monitor the main phone line and respond to any inquiries from Cottage Health.
- Refer to Team Leader or resident’s provider for crisis and suicide risk assessment, provide appropriate interventions and consult with supervisor, complete critical incident reporting with 24 hours as required per contract and PATH policies.
- Provide crisis intervention services focused on enhancing the clients’ ability to independently problem solve, use effective coping skills, and manage own care.
- Use evidence-based practices in service delivery such as intensive case management, Motivational Interviewing and Harm Reduction.
- Complete progress notes on every face to face/ telephone contact with client or collateral contact by the end of each shift.
- Meet with Team Leader weekly for supervision.
- Homeless services, case management, mental health and permanent supportive housing experience preferred.
- Flexible work schedule to include some evenings and/or weekends
- Must possess a high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.
- Ability to provide community and in-home based services on a regular basis.
- Current Basic Life Support (BLS) certification.
- Strong communication skills.
- Highly motivated self- starter with the ability to coordinate multiple projects simultaneously in a high pressure environment.

The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
