Rapid re-housing is an intervention, informed by a Housing First approach that is a critical part of the homeless crisis response system. Rapid re-housing rapidly connects individuals and families experiencing homelessness to permanent housing. Services include time-limited financial assistance and targeted supportive services. Rapid rehousing programs help individuals and families living on the streets or in emergency shelters solve the immediate challenges to obtaining permanent housing while reducing the amount of time they experience homelessness and linking to community resources that enable them to achieve long-term housing stability.
The case manager must have experience working with individuals that are homeless and/or human services. Knowledge of Public Housing Authority and housing subsidy is preferred (ie: Section 8, Shelter Plus Care, and VASH). This Case Manager position will be working under the direction of the Program Manager of Veteran and Community Integration. This person will provide client advocacy, case management, benefit establishment, linkage to mental health/substance abuse services and all other supportive services. The Case Manager will work on a team and will provide individualized client support by helping each client develop a plan to address their barriers, increase their income, and maintain and sustain permanent housing. The position is expected to use evidence-based practice models such as Trauma Informed Models and Motivational Interviewing.

- Perform assessment; develop a plan of intervention that addresses identified need for housing, ongoing case management services, substance abuse treatment, mental health services, and coordination of needed community services
- Conduct intake and individualized needs assessment (basic eligibility screening) for all clients collaborate with clients to develop Individualized Service Plans (ISP) that address barriers to obtaining services/housing and appropriate health care needs.
- Communicate with established clients on a weekly basis to review, evaluate and support completion of ISP plans.
- Complete case notes and data entry into the Homeless Management Information System (HMIS).
- Maintain documentation standards as set forth by the program contract, HIPAA standards, and PATH policies
- Provide ongoing case management support with benefits establishment (GR, SSI, TANF, Veterans pension, etc..), obtaining ID, birth certificate, social security cards, provide employment linkages and assistance, housing referrals and health related assistance as identified on the ISP.
- Assist with housing applications, complete supportive and subsidized housing paperwork, survey rental market for affordable housing, and advocate with prospective landlords, and identify appropriate permanent housing options for guests.
- Provide on-going information, referrals, linkages, and advocacy for all other identified needs.
- Consult with supervisor to conduct crisis and suicide risk assessment and provide appropriate interventions, complete critical incident reporting as required per contract and PATH policies
- Provide crisis intervention services focused on enhancing the clients’ ability to independently problem solve, utilize effective coping skills, and manage own care.
- Collaborate with coworkers and community partners as needed in order to deliver quality and uninterrupted services to veterans
- Attend and participate in all meetings and assigned trainings
- Homeless services, case management, mental health and permanent supportive housing experience preferred
- Flexible work schedule to include some evenings and/or weekends
- Ability to work independently and within a team environment and exercise mature judgment
- Ability to provide community and in-home based services on a regular basis.
- Strong communication skills.
- Highly motivated self-starter with the ability to coordinate multiple projects simultaneously in a high pressure environment.

- Reliable transportation
- Updated tuberculosis test
- Successful completion of background screening
- Able to obtain First Aid/CPR certification as provided by PATH
- Driving is an essential function of this position
- Must have Valid CA Driver's License
- Must provide proof of insurance coverage
- Must be able to qualify for PATH insurance coverage
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
