This position will provide housing and supportive services to homeless individuals residing in the Santa Clara County. Working as part of a multi-disciplinary team, this position is responsible for providing outreach, field based case management, employment, and housing location and retention services to clients enrolled in the program. Services will be strengths based, focused on reducing barriers to housing stability, community integration, and overall wellness. Services will be individualized, client centered, and designed to meet medical necessity. This position will be stationed at PATH San Jose office, but work predominately in the field, providing services in clients’ homes, shelters, on the streets, in health facilities, or wherever clients are residing throughout Santa Clara County.

- Provides case management services in the areas of, but not limited to: street outreach, case management, housing location and stabilization, independent living skills, community integration, employment linkage, benefits establishment, linkage to community providers for substance abuse, primary health care, and all other services needed to assist clients in reaching their goals
- Attend clinical, administrative, and case conferencing meetings as dictated by the program supervisor
- Provide strengths-based case management and service coordination services designed to assist clients in obtaining and maintaining stable housing
- Coordinate recovery-focused and community integration activities designed to build self-sufficiency skills, stabilize clients in housing and promote overall health and wellness
- Develop individualized service plans in collaboration with client addressing short term and long- term goals
- Provide on-going case management to assess progress and ensure service plan outcomes are met or changed as needed
- Conduct crisis and risk assessments in consultation with supervisor
- Provide crisis intervention services focused on enhancing the clients’ ability to independently problem solve, use effective coping skills, and manage own care
- Use evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention and Housing First practices
- Complete intake, assessment, individual service plans in coordination with client and within the documentation standards set the City of San Jose, Santa Clara County and PATH
- Assist and support program documentation standards of staff in accordance with PATH, and funding source standards and requirements, and documents in required program
- Maintain documentation standards as set by PATH policies, including maintaining compliance with HIPAA policies and practices
- Complete progress notes on every face to face/ telephone contact with client or collateral contact by the next business day
- Enter all data on time and correctly to support program evaluation and outcomes tracking
- Ensure proper and timely documentation of services including written case notes using the GIRP formats
- Maintain and execute confidential information according to HIPAA standards
- Experience working in homeless services, mental health, substance use/co-occurring disorder, or with vulnerable populations providing intensive case management services preferred
- Substance abuse certification a plus
- Permanent supportive housing experience preferred
- Flexible work schedule to include some evenings and/or weekends as needed
- HMIS experience and/or Medi-Cal documentation experience preferred
- Use a non-judgmental, respectful approach working with individuals who present for services with urgent multiple case management and health needs
- Ability to work independently and within a team environment and exercise mature judgment
- Ability to provide community and in-home based services on a regular basis
- Strong communication skills
- Highly motivated self- starter with the ability to coordinate multiple projects simultaneously in a high- pressure environment
- BS with at least one year of experience working with vulnerable populations, mental health, PSH or in ACT or homeless services
- Education can be substituted by years of experience

- CPR/First Aid training
- Driving is an essential function of this position
- Must have Valid CA Driver's License

The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
