Summary:

This position will work to support the Director and Associate Director in the operation of programs falling within the ICMS umbrella, and starting up new programs that come online. This position will provide direct participant services and carry a reduced case load, as well as have clinical supportive/administrative responsibilities of special programs. This position will also oversee documentation standards, ensuring high data integrity and support in the completion of required reports.

This position will liaise with funders, other community providers and landlords to identify available units and facilitate case management and psychosocial needs to stabilize participants in housing, reduce functional, health and mental health barriers, and promote recovery and community integration. This position will work out of the PATH Regional Center in Los Angeles providing in office and field based support in participants’ homes, shelters, on the streets, in DHS facilities or wherever participants are residing throughout LA County.

  • Case Management Support – Carry a reduced caseload of higher acuity participants. Provide field based/in office clinical support and supervise case managers. Complete Quarterly Supervision Assessment Tools and annual evaluations with staff. Submit housing related documentation to partners. In conjunction with case managers, provide guidance to newly housed participants, ensuring a supportive transition into permanent housing. Coordinate and participate in group outings and events with community partners.
 
  • Program Management & Oversight- In collaboration with supervisors, provide oversight and administration of program contracts to ensure successful implementation, coordination of participant services, achievement of program performance measures, and evaluation of services provided. In collaboration with supervisors and Quality Assurance team, implement policies and procedures, and document as required by contractual obligations and agency practices. Ensure that program is in compliance with contracts, partnership agreements, and all other funding requirements, program goals, and stakeholder expectations. Assist w/team and all-staff meetings, providing leadership and ensuring agendas are created ahead of time. Review and approve timecards. Train new staff and provide onboarding support which includes PATH payroll system, program and field case management approach, documentation expectations, and file structure. Track PATH and funder required program trainings. Process referrals and enroll into HMIS program as needed.  Administer, track, and provide oversight on appropriate use of participant services funds.
 
 
  • Program Outreach Represent PATH and its ICMS program at community and provider meetings and events. Source housing in the Los Angeles areas for ICMS Case Managers, identifying and acquiring units for participants, paying special attention to service needs. Track new housing and assist with matching. Assist with finding resources and creating alliances for the benefit of harder to serve clients such as undocumented, reentry, etc.
 
  • Reports/ Contracts and Compliance – Oversee completion and submission of program and contract reports, as required by PATH, DHS, and other partners. Complete, review, and submit incident reports. Lead staff training on contract scope, goals, milestones, timelines, and other obligations. Train & support staff in documentation/filing requirements, including but not limited to HMIS and CHAMP. Track notes, ISP’s, psychosocials, and housing subsidy requirements. Review files w/case managers, and lead internal program quarterly audits. Complete monthly QA audit (5 files/month) to ensure high data integrity. Attend and serve as leads at all monitoring site visits.
 
  • Program development and evaluation- Support Director and Associate Director in start-up of new programs, and oversee new program operations. Function as a liaison between funder and program to ensure system level barriers are reduced, innovative strategies and interventions are implemented and in alignment with contract scope of work and PATH’s integrated service approach. Collaborate and work alongside the evaluation team to ensure program level data is used to assess and manage program staff, and program and participant outcomes.
 

Qualifications

  • Master Degree in Social Work (MSW) preferred.
  • At least two years of experience providing direct mental health or intensive case management services.
  • Extensive experience working in homeless services or working with vulnerable populations.
  • Extensive experience in staff supervision, program development and implementation and development of policies and procedures.
  • Demonstrated experience with program start-up preferred.
  • Extensive experience working with program funders.
  • Experience analyzing program procedures and practices to identify barriers and impediments to achieving goals and making recommendations for course-correction.
  • Knowledge of program management, experience preparing and monitoring program budgets.
  • Effective organizational, time management, detail oriented, and communication skills. Includes speaking, listening, and writing in a clear, thorough and timely manner using appropriate and effective communication tools and techniques.
  • Strong ability to effectively resolve and cope with immediate crisis situations.
  • Proven ability to work independently, effectively as an individual and part of a team in a fast- paced environment.
  • Ability to represent the interests of the agency, interact effectively with a diverse group, and be comfortable building successful collaborative relationships with program participants, staff, and other stake holders.

Work Environment
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
