Team Lead (JR 1271) Veteran Services - VASH

Summary:

The VASH Team Lead will work under the direction of the Regional Clinical Supervisor of VASH to provide administrative supervision, oversight and case consultation for VASH staff. The VASH  Team Lead will manage a reduced caseload to provide administrative support and supervision to the team in the following ways: attend and present at community meetings, outreach to local businesses and organizations, develop weekly and as needed progress reports, provide administrative oversight of VASH team region, including but not limited to, documentation quality assurance, VASH billing, program reporting requirements, and employee annual evaluations. VASH Team Leads located in the outlying service area of the Antelope Valley will have an increased level of independence and responsibility.
The VASH Team Lead, under the direction of their supervisor, provides client advocacy, case management, benefit establishment, linkage to Mental Health/Substance Abuse services, linkage to stable housing and all other supportive services as needed. The VASH Team Lead will provide individualized client support by helping each client develop a plan to address their barriers, increase their income, and maintain and sustain permanent housing. Uses evidenced based practice models such as ACT (Assertive Community Treatment), CTI (Critical Time Intervention), trauma-informed models and motivational interviewing for substance abuse treatment.

Social Services

  • Provide guidance to staff on a regular basis. Conduct monthly staff evaluations utilizing PATH’s employee evaluation tool.
  • Conduct team meetings weekly to review cases, provide training and education and discuss any administrative issues.
  • Facilitate daily conference call with staff to review cases and ensure that services align with contract obligations.
  • Provide regular training to staff one on one and work along side staff in the course of their work.
  • Provide on-going constructive feedback to staff.
  • Carry a small client case load or work in conjunction with staff on more challenging and complex cases as necessary.   Complete all client related documentation as required.
 
 

Administrative

  • Ensure that all program data is accurate and entered into the required program documentation system as contractually required.
  • Responsible for generating monthly, quarterly, annual and ad-hoc reports for submission to management and funders in a timely manner.
  • Responsible for monitoring program outcomes and informing program director of any needs or changes that may be necessary.
  • Operate as the primary contact person for all program matters, keep supervisor informed of requests. This includes program monitoring by funders, responding to specific program inquires from management and other stakeholders.
  • In collaboration with supervisor, conduct employee performance management including correction action planning of direct report staff.
 
 

Qualifications

  • The VASH Team Lead must have extensive experience working with individuals who are chronically homeless and/or homeless Veterans.
  • Knowledge of Public Housing Authority and housing subsidy is ideal (ie: Section 8, Shelter Plus Care, and VASH).
  • Excellent verbal and written communication skills.
  • Strong organizational, planning and time management skills.
  • Excellent interpersonal skills, ability to work in a variety of settings with culturally-diverse persons and communities with the ability to be culturally sensitive and appropriate.
  • Strong ability to effectively resolve and cope with immediate conflict and/or crisis situations.
  • Proficient use of computers, basic office software and any other database software(s) used to track work outputs.
  • Excellent leadership skills with a hands-on, lead-by-example work style.
  • Knowledge of community resources.
  • Maintain and execute confidential information according to HIPAA standards.
  • Flexible work schedule to include some evenings and/or weekends as needed. 
  • Must possess a high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.
  • Ability to work independently and within a team environment and exercise mature judgment.
  • Ability to provide community and in-home based services on a regular basis.
  • Highly motivated self- starter with the ability to coordinate multiple projects/ tasks simultaneously in a high pressure environment.
 
Required Qualifications
  • Masters of Social Work
  • Employment Eligibility Verification
  • Updated tuberculosis test
  • Successful completion of background screening
  • CPR/First Aid training
  • Driving is an essential function of this position
    • Must have Valid CA Driver's License
    • Must provide proof of insurance coverage
    • Must be able to qualify for PATH insurance coverage
 

Work Environment

The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.

While performing the duties of this job, the employee is frequently required to stand, sit, walk, stoop, talk, hear, reach above and below shoulders; use hand and finger dexterity, keyboarding and making and receiving telephone calls. The employee may be required on occasion to lift and or carry up to 20 lbs.
