Summary:

Under the supervision of the Associate Director, the Program Coordinator/Navigator III’s primary role is to support the program manager to ensure clients are well served and community requests are responded to effectively by coordinating and supporting a full range of activities related to street outreach. The Program Coordinator will participate in daily street outreach to the homeless clients in the San Jose community. Utilizing the PATH street outreach integrated care approach, navigators will provide both outreach/engagement services and brief case management interventions to quickly assess and link clients to longer term housing, health, mental health and other supportive services. The Navigator will attend community meetings, conduct outreach to local businesses and organizations, provide referrals and linkages to necessary services, and transportation to clients accessing services. The Navigator will work as part of a Street Outreach Team to identify homeless trends and work closely with the community to identify solutions.

Supportive Services

  • Conduct daily street outreach to develop a relationship/build rapport that will assist clients with addressing barriers to obtaining services/housing and appropriate health care needs.
  • Conduct and complete all necessary paperwork to track client progress (daily logs, ISP, etc.).
  • Meet with established clients on a weekly basis to review, evaluate, and support completion of their goals.
  • Respond within 24 business hours to requests for street outreach services and send follow upinformation to requestor and supervisor.
  • Conduct a VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) on all new clients to connect them to the proper intervention and services. Ensure all data is entered in a timely manner. Fill in information on internal tracking spreadsheet.
  • Provide street case management services by bringing services to them in the community with the goal of linking/connecting them to long term supportive services. Navigator will assist the client with benefits establishment (GR, SSI, TANF, Veterans pension, etc.), obtaining ID, birth certificate, social security cards, provide employment linkages and assistance, housing referrals and health related assistance as identified on the ISP.
  • Assist with housing applications, complete supportive and subsidized housing paperwork, survey rental market for affordable housing, and advocate with prospective landlords, and identify appropriate permanent housing options for clients.
  • Build relationships and collaborate with community substance abuse and mental health providers to ensure seamless referral and services from street outreach to longer term mental health services.
  • Work collaboratively with local law enforcement and City of San Jose interim service providers to ensure transition from streets into interim housing is collaborative and meeting the client and communities’ needs.
  • Provide on-going information, referrals, linkages, and advocacy for all other identified needs.
  • Creatively use and develop community resources to broker and link clients to services.
  • Provide a high quality customer service environment for all clients.
  • Participate in staff meetings and training as assigned by your supervisor.
  • Provide administrative support including data analysis and reporting.
  • Support leadership through contract growth and project developments including the planning and coordination of projects and its activities and the on boarding and training of new staff, interns, and volunteers.
  • Coordinate team members to ensure community requests are addressed in a timely manner
  • Guide and offer insight to client engagement strategies.
  • Help build and maintain positive relations within the team and with external parties, foster positive team culture and practice professional leadership.
  • Evaluate reports, decisions, and results of the Street Outreach Team and recommend new approaches, policies, and procedures to effect continual improvements in operations and efficiency.
  • Strengthen PATH San Jose’s landlord networks and engagement strategies, build a landlord database, and facilitate the capacity building of case managers to increase housing placement successes
  
Documentation
  • Maintain documentation standards as set forth by the program contract and PATH policies.
  • Complete paperwork on every face to face / telephone contact with client or collateral contact by the next business day at 5pm.
  • Ensure all program data is accurate and entered into the required program documentation system as contractually required. (i.e. HMIS, internal spreadsheet, CES, etc.)
  • Prepare case-related reports including but not limited to: demographics, outcomes, successes and challenges.
  • Generate client data for reporting.
  • Complete follow-up and retention services, as necessary, and provide back-up documentation in client file.
  • Maintain complete and accurate documentation of service objectives and outcomes as well as other services in accordance with Federal, State, County and PATH guidelines.


Qualifications:

  • BA in related field or 2 year of experience in street outreach with highly vulnerable populations, particularly those with health, mental health, and substance abuse issues.
  • Demonstrated knowledge and or experience with advanced and evidenced based interventions, including engagement and rapport building with hard to reach clients, Harm Reduction and Motivational Interviewing strategies, Housing First, non-violent crisis intervention techniques and case management interventions effective with a vulnerable population
  • Able to work independently and as part the team and exercises mature and professional judgment.
  • Strong written and verbal communication skills.
  • Good problem solving and conflict resolution skills.
  • A highly motivated self-starter and ability to coordinate multiple projects/tasks at once.
  • Knowledge of maintaining and executing confidential information using HIPAA standards.
  • Computer skills with proficiency in Microsoft Office software.
  • Flexible, adaptable and have the capability to work in a fast paced, professional environment.
  • Able to work alternative work schedules, evening and weekends as needed for operations of Street Outreach and at the request of management.
  • Maintain regular, consistent attendance.
 

Required Qualifications

  • CPR/First Aid training 
  • Driving is an essential function of this position

Work Environment
The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
