The Outreach Case Manager, II will participate in daily street outreach to homeless clients in the Manchester Square community. Using the PATH Street Outreach Integrated Care approach, Outreach Specialist will provide outreach and engagement services to conduct brief case management interventions to quickly assess and link clients to long term housing, health, mental health and other supportive services. The Outreach Specialist will attend community meetings, develop relationships with local businesses, organizations, and service providers. Outreach Specialist will refer clients for necessary services and offer transportation assistance.
The Outreach Specialist will work as part of a Street Outreach Team to identify solutions to trends within the homeless community.
This position is unique in that the area is on a time restraint, with demolition and construction eliminating the area by April 2018. This position requires not only experience but creativity as almost 250 residents will be displaced into surrounding communities. This position will require immediate action regarding interventions, taking a lead in community engagement, developing specialties through intense training and detailed documentation. This position requires concentrated work with an MSW, understanding symptomology and applying correct clinical interventions.

- Supportive Services - Conduct daily street outreach to develop relationships and build rapport with clients in order to assist them with obtaining services, (housing and appropriate health care needs). Complete all necessary paperwork to track client progress, (daily logs, ISP, etc.). Meet with established clients on a weekly basis to review, evaluate, and support goal completion. Respond within 24 business hours to requests for street outreach services and send follow-up information to the requestor and the supervisor. Conduct VI-SPDAT, (Vulnerability Index- Service Prioritization Decisions Assistance Tool), on all new clients in order to connect them to proper intervention and services. Document results, complete match initiation form, and submit all paperwork to CES Coordinator. Provide street case management services by connecting homeless clients to long-term supportive services. Assist client with the establishment of personal benefits including (GR, SSI, TANF, Veterans Pension, etc.). Help client to obtain various forms of identification including birth certificates and social security cards. Provide employment assistance, housing referrals, and health related assistance, as identified on the ISP. Assist with applications for supportive and subsidized housing and prospect potential locations for affordable rental housing. Build relationships with prospective landlords, and identify appropriate permanent housing options. Collaborate with community substance abuse and mental health providers to ensure seamless referral services. Work with local law enforcement and the local service providers to ensure a smooth transition from street living to interim housing. Provide ongoing information, referrals, linkages, and advocacy for all other identified needs. Creatively use and develop community resources to broker and link clients to services. Provide a high quality customer service environment for all clients. Participate in staff meetings and training as assigned by supervisor.
- Documentation - Maintain documentation standards as set forth by the program contract and PATH policies. Complete paperwork on each face to face and telephone contact with the client, or with any collateral contact, by close-of-business on the next working day. Ensure all program data is accurate and entered into the appropriate program documentation system as contractually required. (i.e. HMIS, internal spreadsheet, CES, etc.) Prepare case-related reports including but not limited to: demographics, outcomes, successes and challenges. Generate client data for reporting. Complete follow-up and retention services, and provide back-up documentation in client file. Maintain complete and accurate documentation of service objectives and outcomes, as well as other information in accordance with Federal, State, County and PATH guidelines.
- At least 2 years of experience working with chronically homeless population
- Demonstrated knowledge and experience with advanced and evidenced based intervention

- Availability to work evening and weekends as requested by management, and based on the operational needs of the Street Outreach program
- Strong written and verbal communication skills
- BA in related field or 2 years of experience in street outreach with highly vulnerable populations, particularly those with health, mental health, and substance abuse issues
- Substance Use Counselor or similar credential preferred
- Ability to maintain regular, consistent attendance

- Reliable transportation, and valid California Driver’s License
- Updated tuberculosis test
- Successful completion of background screening
- Ability to obtain CPR/First Aid certification through PATH
- Driving is an essential function of this position
