The Direct Care Monitor will work collaboratively with fellow monitors, the Program Manager of the Interim Program and the Associate Director of West Los Angeles to oversee the PATH West Los Angeles Interim. The program focuses on veterans using the Harm Reduction model. Case management is recovery- oriented and integrates evidenced-based practices to assist individuals with long-term housing sustainability. The Direct Care Monitor works as part of a team with residents on benefit enrollment and employment opportunities to increase income and linkages to community resources to decrease social isolation while addressing behavioral health and substance abuse needs throughout the process. The Direct Care Monitor follows established low barrier guidelines to create a safe and stable environment for veterans entering the program. The Direct Care Monitor makes referrals as needed to other programs, transports veterans to appointments as requested in agency van and ensures veterans have access to supportive services as identified in the ICP (individual and group counseling, social skills training, family counseling, etc.). This position may also be responsible for assisting with navigator duties that include meal preparation, answering phones and assisting with general facility needs as they arise.

- Work in collaboration with the Program Manager, Associate Director and Director to create and maintain Harm Reduction Plans
- Monitor individualized care plan in collaboration with client and Program Manager addressing short term and long term goals, including necessary interventions with regard to housing, substance treatment, mental health services, and any other necessary assistance
- Assist in ongoing case management support to assess progress and ensure treatment plan outcomes are met or adjusted, with the Program Manager
- Meet weekly or more with veterans to provide supportive case management services addressing mental health, functional, and substance use barriers to help clients obtain and maintain housing as directed by Program Manager
- Refer to Program Manager and Associate Director for crisis and suicide risk assessment, provide appropriate interventions and consult with MSW and LCSW supervisor, complete critical incident reporting with 24 hours as required per contract and PATH policies.
- Provide crisis intervention services focused on enhancing the clients’ ability to independently problem solve, use effective coping skills, and manage own care, document appropriately
- Provide intentional case management supports that move the client from a high intensity level of service to a more independent, recovery-focused level of service as appropriate as directed by Team Leader
- Use evidence-based practices in service delivery such as intensive case management, Motivational Interviewing and Harm Reduction.
- Conduct random urine analysis; provide the Safe Room as an option for observation following established guidelines.
- Support veterans who have difficulty with sobriety in making safer choices (ex. needle exchange).
- Identify permanent supportive housing units and work closely with individuals to move from street to home as quickly as possible using a Housing First approach.
- Transport passengers and their personal possessions to destination in a safe and courteous manner.
- Operate the company van in accordance with all laws, regulations and company policies to ensure a safe and pleasant environment.
- Maintain documentation standards as set forth by the program contract and PATH policies, ensuring confidentiality of information per HIPAA standards.
- Complete progress notes on every face to face/ telephone contact with client or collateral contact by the next business day at 5pm.
- Complete End of Shift reports, incident reports, bed census and other supportive documents as designated by the VA per contract
- Homeless services, case management, mental health and permanent supportive housing experience preferred.
- Flexible work schedule to include some evenings and/or weekends
- Must possess a high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.
- Ability to provide community and in-home based services on a regular basis.
- Current Basic Life Support (BLS) certification.
- Strong communication skills.
- Highly motivated self- starter with the ability to coordinate multiple projects simultaneously in a high pressure environment.

The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
